Patients who:
Want predictable follow-up and refills
Are on stable long-term medications
Prefer structured care instead of one-off scheduling
Are not receiving ongoing compounded hormone management
Depending on your current treatment structure, you may opt in or opt out.
What’s Included
- Up to 2 visits every 6 months (15–30 min)
- Stable medication refills
- One routine lab panel per 6 months (if clinically appropriate)
- Proactive refill oversight
- Continuity and care coordination
What’s NOT Included
- Urgent care visits
- Antibiotics for acute illness
- New or complex condition workups
- Extensive portal medical decision-making
- Emergency services
